Referrals to the ADHD Foundation

If you are working with a child or young person who is presenting with characteristics of ADHD, you may make a referral to Liverpool ADHD Foundation by following our Referral Pathway:

Child or young person presenting with characteristics of ADHD?
Does the child or young person live in Liverpool?
Does the child or young person have a diagnosis of ADHD by a Community Paeditatrician, Educational Psychologist or Psychiatrist within the last two years?
Complete our Referral Form (see below) and return to our Toxteth Office	Contact Liverpool ADHD Foundation to make an appointment with our Information, Advice & Guidance Team
Liverpool ADHD Foundation will contact you to discuss this referral.	Liverpool ADHD Foundation will assist you to identify the support needs of the child or young person.
